Health Exams, including immunizations

Per the School Code of Illinois, a new physical examination is required for entrance into Pre-Kindergarten, Kindergarten and 6^th Grades and/or when transferring from an out of state school.

All Da Vinci Academy students must have a current Illinois Department of Public Health Well Child Exam Form on file before their first day at Da Vinci. Pediatricians typically have this form in their offices or you can click here.

All immunizations must also be up to date unless an objection is formally on file with the school office.

Dental Exams

Students currently enrolled in grades Kindergarten, 2nd and 6^th must have an oral health examination performed by a licensed dentist. For the 2010-2011 school year, this form must be in the school office before May 15, 2008 and must not be more than 18 months old.
