The Da Vinci Academy application for admission includes the following components:

1. Application Form

Application for admission during the 2011-12 school year
Application for admission in the 2012-13 school year
If possible, please provide a photo of the applicant to help us keep faces with names during the admissions process
Parents return the completed form to Da Vinci Academy

2. $50 non-refundable application fee

Please include a check made to Da Vinci Academy when submitting the application form

3. Parent Questionnaire (please click below to download the appropriate PDF file)

Pre-K (half-day PK3 and full-day PK4)
Kindergarten to 4th Grade
Middle School Grades 5-8
Parents return the completed questionnaire to Da Vinci Academy (hand-written or typed)

4. Lake Michigan Association of Independent Schools (LMAIS) Recommendation Form

Pre-K Teacher Recommendation
Grades K-1 Teacher Recommendation
Grades 2-5 Teacher Recommendation
Grades 6-12 English Teacher Recommendation
Grades 6-12 Math Teacher Recommendation
Parents provide the previous year's or the current year's teacher with the appropriate form, and the teacher returns the completed form directly to Da Vinci Academy

5. Parent copies of IQ test results, grades, school reports, achievement tests, etc.

IQ testing (Wechsler exam, either the WISC-IV or WPPSI) required for applicants to grades 1-8
Schedule testing using our list of recommended IQ testers
Parents submit IQ results and the tester's written report to Da Vinci Academy
Parents can submit copies of grade reports, achievement tests, or other testing that they have at home

6. Request for Student Records

Families who enroll in the school should be prepared to provide current student physical and health/immunization records.
